PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
?? Performs all responsibilities/duties required by the pharmacy as defined by the scope of service to ensure that the unique nature of the patient is addressed.
?? Monitor Provider order entry for medication interactions, over/under dosing, contraindications, pregnancy, allergies and appropriateness for therapy.
?? Carry out daily requirements for anticoagulation management, renal dosage management, TPN formulation ordering and management, antibiotic stewardship, formulary substitution management, IV-to-PO transition, daily monitoring and ordering of all pertinent laboratory data necessary for pharmacy dosing/monitoring of medications.
?? Respond to and follow up on daily Pharmacotherapy consults.
?? Issue and order controlled substances and maintain records as required by State and Federal Law.
?? Able to prepare all chemotherapy and IV additive medications using sterile technique and according to the policies and procedures set by the pharmacy department.
?? Documentation of adverse drug reactions, all clinical activities and interventions and consultations.
?? Training of Pharmacists, nurses, technicians and other staff as requested.
?? Patient counseling as requested.
?? Participation in formulary review and hospital committees as requested.
?? Responsible for final check on products prepared by the pharmacy department, including first dose mediations, bulk products, automated dispensing machine refills, and IV preparations.
?? Assumes "charge pharmacist" responsibilities as required, for the coordination of weekend and evening shifts which include supervising the PRN pharmacists and supportive personnel, assigning work and delegating tasks and responsibilities, and for handling problems that arise during the absence or unavailability of the director of pharmacy services.
?? Responsible for ensuring nursing station checks are completed to verify that medications are properly and safely stored, that excess and out dated drugs are removed, and that information regarding drug usage and administration is communicated to the nursing staff. Responsible to function as a pharmacy liaison to nursing units for the purpose of handling routine, day to day issues.
?? Completes all required education and safety training as defined by department requirements.
?? Participates in providing an environment conducive to learning for students from the professional schools utilizing the hospital for clinical experience.
?? Assists in the training of other personnel.
Qualifications
KNOWLEDGE AND SKILLS REQUIRED:
?? Ability to work independently with minimal direct supervision.
?? Ability to work cooperatively with health system and pharmacy staff.
?? Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
?? Ability to communicate effectively (orally and written).
?? Ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, oncology, obstetric, and transitional care patients.
KNOWLEDGE AND SKILLS PREFERRED:
?? Ability to utilize all methods of drug information and reference material, written or electronic in order to provide timely information to nurses and providers
EDUCATION AND EXPERIENCE REQUIRED:
?? BS in Pharmacy from accredited school
?? Minimum one year of Hospital Pharmacy experience
EDUCATION AND EXPERIENCE PREFERRED:
?? Pharm.D., Masters Degree, Residency
?? Any pertinent Board Certification (i.e. Pharmacotherapy)
?? Five years of Clinical Hospital Pharmacy experience preferred
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
?? Current State of Florida Pharmacist License

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
